At Jordan Family Health our mission is to provide the highest quality, cost effective care to every patient, meeting not only his or her medical needs, but emotional needs as well.
We try to accommodate our patient's busy lives by offering Urgent Care hours Monday - Thursday 8:30am - 9:00pm, Friday 8:30am - 5:00pm and Saturday 8:30am - 12:30pm. We are now accepting new patients and same day appointments.
Welcome Dr. Yavelak
Dr. Yavelak joined our practice in June 2013. Prior to joining Jordan Family Health, Dr Yavelak worked in Nashville, Tennessee at Tristar Medical Group: Blackman Family Physicians. He was privileged at Stonecrest Medical Center in Smyrna, TN, and he also had a clinic in Murfreesboro, TN.
Dr. Yavelak practiced Family Medicine as a Lieutenant Commander in the US Navy Medical Corps. Following completion of his residency at the Family Practice Department at Naval Hospital Jacksonville in 2005, Dr. Yavelak served as the medical officer for the 12th Marine Regiment in Okinawa, Japan until 2007. He then practiced at the Naval Hospital Oak Harbor until October, 2010, also acting as the director of the Urgent Care Clinic since 2009. He received his medical degree at the Medical College of Ohio and is Board Certified in Family Medicine.
In his spare time, Dr. Yavelak enjoys playing with his two sons, making fine furniture, golfing, and skiing.
Specialty Care at Jordan Family Health
To better serve our patients needs, we recently opened the Specialty Care Network at Jordan Family Health. We have teamed with Old Farm Obstetrics and Gynecology, Salt Lake City-based ENT Specialists and Utah Orthopedic Associates to bring OB/GYN, ENT and Orthopedic services to the south end of Salt Lake County.See all our specialty services.

Our clinic is working towards accreditation as an NCQA Patient Centered Medical Home (PCMH). What does this mean to you? It means a health care setting that facilitates a partnership between you, your personal physician, and when appropriate, your family.
